New Patients

Joining the practice is easy. All eligible people living in the practice’s catchment area can register with the surgery as a regular patient; you can check the catchment area below.

If you do not have an address of your own, please state upon the new patient form that you do not have an address. You do not need to provide proof of your address, photographic ID or immigration status to register with us. After you have completed and submitted your registration forms your registration will be processed.

We may ask for confirmation of identity, addresses etc to help us trace your previous medical records, but you do not need to provide these unless we ask for them.


How to Register

New patients wishing to register will not be discriminated against on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or mental condition.

If you would prefer to print off and complete our registration form to bring to the surgery, please use the below link. For New Born Babies – please bring Red Book &/or Birth Certificate in to the surgery.  

After you have completed the form you must attend the surgery within 7 days with one form of identification and proof of address alongside the registration form.

– Birth Certificate

– HC2 Certificate

– Rough sleepers’ identity badge

– Hostel Registration/mail forwarding letter

Until we have seen your identification you will not be registered at the practice.

We will also need you to bring the following when you come in to register:

  • History of vaccinations for all children.

Temporary Residents

If you are not registered with a doctor but need to see one, you can receive emergency treatment from a surgery for 14 days. After 14 days you will need to register as a temporary patient or permanent patient.

You can request a Temporary Resident Form from your surgery. You can register as a temporary patient if you are in the area for longer than 24 hours but less than three months.

Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received. For information for overseas visitors please see here
